(Hybrid) Community Support Specialist (24105)

The purpose of this position is to provide support for the Mentoring & Oversight for Developing Independence with Foster Youth (MODIFY) Program at the WVU Center for Excellence in Disabilities (CED).

As a (Hybrid) Community Support Specialist, you will provide follow-up services to youth in transition from foster care to adult independence. The primary emphasis is the coordination of services to youth exiting foster care. Service coordination provided to this group of young adults will ensure that system linkages are in place for supporting these youth in the life domains of housing, employment, education, finance, physical/mental health, family and socialization. The goal of these services is to promote self-sufficiency and a successful transition to independence for former foster care youth.

The purpose of this position is to perform clerical and administrative functions following established policies, procedures, and methods in support of the Principal Investigator, Manager, and staff of the MODIFY Program.

This position provides program, as well as direct client support services. Responds to inquiries and independently resolves matters of low to moderate complexity. Responsibilities require some judgment, resourcefulness, and originality, but diverse guides and precedents are usually available to apply or adapt to the circumstances. Work effort has considerable impact on client services, program and project image, with potential legal liabilities.

TBI Community Social Worker - Wood, Marshall, Tyler, Doddridge, Harrison, Pleasants, Ritchie, Gilmer, Wirt, Calhoun, Roane, Jackson and Mason County (25483)

This position will provide resource coordination services to individuals with traumatic brain injury so they can effectively access available services; and, to facilitate a seamless continuum of services across agencies. This position will serve as a member of the State of WV Traumatic Brain Injury (TBI) Program; conduct assessments; develop intervention recommendations; link to resources; identify unmet needs and barriers to accessing effective TBI services; collect data to support development of a funding stream for statewide brain injury services and supports; conduct regional outreach to identify and assist individuals with TBI in need of support services; and, provide training and technical assistance to improve service providers capability to offer effective, person-centered TBI services. This position provides program, as well as direct internal and external client support services. Responds to inquiries and independently resolves matters of low to moderate complexity. The purpose of this position is to provide resources and support to improve the quality of life for families and children enrolled in the Children with Special Health Care Needs (CSHCN) program through the WV Department of Health and Human Resources (WVDHHR). Children who qualify include children with complex medical conditions and/or children in foster care. We will hire in 3 separate counties of Logan, Wood and Berkeley.
